A California doctor who defrauded Medicare of more than $45 million has been sentenced to 15 years in federal prison. The Justice Department announced the punishment Friday after prosecutors detailed how the proceeds financed real estate, expensive vehicles and opulent collectibles.
Violetta Mailyan, 45, of Glendale, was convicted in May by a Central District of California jury. Jurors found her guilty on nine counts of wire fraud and three counts of obstruction of a criminal investigation of a health care offense.
🚨FRAUDSTER SENTENCED🚨
A California doctor was sentenced to 15 years in prison for using her medical practice to fraudulently bill Medicare more than $45 million for Botox injection services that were medically unnecessary and never provided. Violetta Mailyan even billed for thousands of injections, representing over $19 million, purportedly provided on days when her clinic was closed. Prosecutors said Mailyan owned and operated Healthy Way Medical Center, a clinic that claimed to offer beauty and cosmetic services. She fraudulently billed Medicare for thousands of Botox injections that were never given or were not provided to treat documented cases of chronic migraines.
Medicare reimburses medical providers for Botox injections when they are medically needed to treat documented chronic migraines. According to prosecutors, Mailyan repeatedly collected payments for injections that did not meet those requirements or simply never happened.

The billing records included more than $19 million worth of injections that Mailyan supposedly administered on days when Healthy Way Medical Center was closed. The extraordinary claims piled up even as the clinic was not operating.
Mailyan also billed Medicare for injections while she was traveling in Mexico, Hawaii, Las Vegas, Pennsylvania and New York at different times. In one instance, she submitted more than $200,000 in claims for services supposedly provided to California patients while she was relaxing in Cabo.
Prosecutors presented a photograph recovered from Mailyan's phone showing a beach scene in Mexico. On that same day, she billed Medicare $34,560 for services she claimed had been provided in Glendale.
As investigators began closing in, prosecutors said Mailyan falsified medical records in an effort to conceal the fraud. She also forged Medicare beneficiary signatures, including the signature of a man who was imprisoned when she claimed to have treated him.
Mailyan submitted falsified records to federal investigators to make it appear that she had seen patients whom she had not actually treated. Prosecutors said she provided those records after receiving a grand jury subpoena seeking medical documents.
The billing data apparently put Mailyan in a league of her own. Prosecutors said she had been identified as an “extreme outlier” among doctors who received Medicare payments for Botox injections.
Mailyan was paid six times as much as the next highest group of providers, all of whom were neurologists. Her operation produced a flood of Medicare money that prosecutors said went toward an expensive collection of property and luxury goods.
Mailyan purchased five pieces of California real estate and two Tesla vehicles, including a Cybertruck. The Justice Department previously said she held four properties in Surfside and Glendale with combined equity of approximately $7.3 million.
Her purchases also included a $12,000 crossbow from the 17th century and a $3,000 painting of Ludwig I, Crown Prince of Bavaria. Those acquisitions joined the homes and vehicles prosecutors tied to money generated through the Medicare scheme.
Along with the 15 year prison sentence, Mailyan was ordered to forfeit more than $16 million in property. The forfeiture covers bank funds, personal property, real property and securities, while she must also pay approximately $32.9 million in restitution to Medicare.
Mailyan had faced up to 20 years in prison for each of the nine wire fraud counts. Each of the three obstruction charges carried a potential sentence of up to five years, although it was not immediately clear when her prison term would begin.
The Justice Department said its fraud enforcement supports President Donald Trump's task force to eliminate fraud. Trump established the task force in March and placed Vice President JD Vance in charge of the initiative targeting fraud across the government.
Trump also announced a $25 million Fraud Whistleblower Incentive Program last month. Under the program, a tip leading to a fraud conviction or significant federal enforcement action can qualify for a reward of up to $30,000, administered by the Justice Department.
During the announcement, Trump said payments would amount to a 1% commission on the fraud uncovered with help from the tip. He also said he would determine who received reward money when more than one person claimed responsibility for providing the information.
The Daily Mail contacted Mailyan's attorneys, Mark Hardiman and Phillip Kanarsh, as well as the Justice Department, for comment. The sentencing closes the courtroom phase with a lengthy prison term, massive forfeiture and a multimillion dollar restitution order.
